So that we may best serve you during this appointment, it will help if you bring the following:

1. Your completed SOA ‘Scope of Appointment’ form, if you have not already provided one to us in the time since your appointment was set. New governmental rules require this form be dated at least three days prior to our appointment, otherwise we have to reschedule for a later date. There are five boxes to mark; two are required and three are optional. We recommend marking all five boxes on the form.
2. A detailed list of your prescriptions, including dosage and frequency, and the name of your preferred pharmacy. It will help very much if you can get this information to us in advance of your appointment. The easiest way to get your prescription list to us is by filling out this form: prescription list. We enter your list into an online tool that tells us how each plan will cover your prescriptions.
3. A list of your medical providers.
4. A blank check (or your bank account number & routing number) so that you may pay your initial premium upon application, should you be prepared to move forward.
