What to bring to your appointment with Tomlin Health Insurance
We look forward to seeing you regarding health insurance! In the meantime, please prepare some information for us so that we can help you find the perfect plan for you and your household. Please see the information below for details:
Medicare Health Plans
So that we may best serve you during this appointment, please prepare as follows:
- Please complete a SOA ‘Scope of Appointment’ form. New governmental rules require this SOA form be dated at least 48 hours prior to your appointment, otherwise we are required to reschedule for a later date. There are five boxes to mark; two are required and three are optional. We recommend marking all five boxes on the form. You’ll also need to sign and date before the form can be completed.
- If you have a Medicare Advantage plan, please complete a detailed list of your prescriptions, including dosage and frequency, and the name of your preferred pharmacy. It will help very much if you can get this information to us in advance of your appointment. The easiest way to get your prescription list to us is by filling out this form: prescription list. We enter your list into an online tool that tells us how each plan will cover your prescriptions.
- If you have a Medicare Supplement plan and a separate, stand-alone PDP Prescription Drug Plan, we ask that you complete a bit of do-it-yourself research. This change is a response we are forced to take due to changing governmental and industry changes. First, we are no longer compensated for selling stand-alone drug plans. Second, we will be far busier this Fall than in prior years due to changes imposed by the Federal government, so we respectfully ask for your collaboration. Please shop for your best PDP on Medicare.gov. Start by clicking this link. For instructions, you can watch this YouTube video that shows how to enter your drug list and shop for and/or enroll in a stand-alone drug plan on Medicare.gov. Note that if you enroll in a PDP via this link, it will automatically cancel your prior PDP.
- A list of your medical providers.
- A blank check (or your bank account number & routing number) so that you may pay your initial premium upon application, should you be prepared to move forward.
