In order to prepare for your appointment, please review this page. If you plan on asking about Part D prescription drug plans or Medicare Advantage, please click this link to complete the Scope of Appointment form prior to your appointment. We do not need the Scope of Appointment form if we only talk about Medicare Supplements.

Our office is located at 1142 Willagillespie Rd. Ste. 10, Eugene OR 97401, in the Delta Triad Professional Building. Our office is located on the lower level of the building. Upon entering the property, take an immediate left toward the lower level parking lot. Suite 10 is located in the center of the building; look for the “Tomlin” tree logo on our front door.

So that we may best serve you during this appointment, it will help if you bring the following:

  1. A detailed list of your prescriptions, including dosage and frequency, and the name of your preferred pharmacy. In our appointment, we will enter your list into an online Medicare.gov tool which tells us how each plan will cover your prescriptions.
  2. A list of your medical providers.
  3. Your NEW Medicare ID card, from 2019 or later. The older cards with your social security number are no longer valid.  If this is a preliminary appointment we can get by without your card.  We can’t enroll you in a plan using the old-style Medicare cards.
  4. A blank check (or your bank account number & routing number) so that you may pay your initial premium upon application, should you be prepared to move forward.

We use a client intake form with all new clients that helps us discover holes or gaps in your coverage. If we haven’t completed one with you, or if you would like to review it (or even complete it in advance) you can do so by clicking here.  It does ask some nosey questions which you may have already covered with your financial advisor. Note that we specialize in post-retirement insurance products. We may have products which you have never seen that could give you a pleasant surprise. After all, “you don’t know what don’t know”.

Lastly, we encourage that you write down any questions you may have for your agent, as they come up, to bring with you on the day of your appointment. Please contact our office with any questions about your appointment.
